Maricopa County Department of Public Health (MCDPH) is committed to the health and well-being of Maricopa County residents. As a part of that commitment, and in agreement with the Arizona Department of Health Services (ADHS) and the Healthy People Healthy Communities Intergovernmental Agreement (HPHC IGA) we are looking to contract and fund several Community Based Organizations (CBO) throughout Maricopa County to support an educational campaign, outreach, and/or health event that will focus on the education, prevention and early detection of any of the four leading causes of chronic disease death in Arizona: Cancer, Alzheimer’s, Chronic Lower Respiratory Disease (CLRD), and Heart Disease. As reported by the Centers for Disease Control and Prevention (CDC) per ARS 36-770 (Proposition 303 Tobacco Tax – Health Education Account Revenue ): Access to early preventative care such as education, screenings and detection is crucial for improving survival rates and enhancing the quality of life for individuals diagnosed with any of the four leading causes of chronic disease death. Health events hosted by trusted community organizations have a positive impact on the communities they serve by providing resources, education, and screenings. They help fill a gap in the healthcare system, especially in communities that are harder to reach due to rural environments, transportation, and language limitations. This program is designed to address this gap by providing a vital service that can lead to early prevention and detection of the four leading causes of chronic disease death, which is associated with better treatment outcomes and reduced mortality rates.
