The Whiteriver Service Unit and Cibecue Health Center require a Contractor to provide proficiency testing and calibration verification samples to meet the Joint Commission’s Quality Standards 01.01.01 through 01.03.01. In order to maintain accreditation with Joint Commission (TJC) and Center for Medicare & Medicaid Services (CMS), the Whiteriver Indian Hospital and Cibecue Health Center Laboratory departments must be enrolled in a proficiency testing program according to CLIA’88 regulations.
The intent of this notice is to gage market capability for competition. The target socio-economic groups are Native American and Alaska Native owned businesses, however since this is market research the government invites all vendors with capability and interest in meeting this need. The full requirements are expressed in the attached Statement of Work. All interested parties should include the following documents:
- IEE Representation Form
- Please complete even if your entity is not registered as an IEE/ISBEE
- Please do not submit previous iterations of this form as they request outdated information that is no longer relevant.
- Capability Statement
- Ensure the statement submitted clearly shows capability to meet this requirement as explained in the SOW.
*Failure to submit the requested documents will result in a ruling of incapatability.
