Specifications include, but are not limited to: A. Pre and Post-Test 1. The Consultant will develop a pre and post-test to be administered during the Student Wellness Leadership Training Program to gather information on the level of knowledge gained by the Student Wellness Leaders. 2. The pre and post-test must utilize a Likert Scale. 3. The pre and post-test must be submitted to LACDMH for pre-approval. 4. The Consultant must submit copies of all completed pre and post-tests to LACDMH. 5. The pre and post-test must include a copy of the LACDMH logo, the County Seal, the MHSA logo, and the Black & African Heritage. B. Community Survey 1. The Consultant will develop a community survey to be administered by the Student Wellness Leaders to attendees of the community Student Wellness Leadership Training. 2. The community surveys must utilize a Likert Scale. 3. The community surveys must be submitted to LACDMH for pre-approval. 4. The Consultant must submit copies of all completed community surveys to LACDMH. 5. The community surveys must include a copy of the LACDMH logo, the County Seal, the MHSA logo, and the Black & African Heritage logo. C. Curriculum for the Student Wellness Leadership Training Program 1. The Consultant must develop the curriculum and materials, written at a 6thgrade comprehension level, for a 12-hour interactive training to become a Student Wellness Leader. 2. The training will provide a foundational understanding of mental health outreach and engagement through the creative modality of art/holisticbased outreach. 3. The Curriculum will also help further develop the five (5) core Social Emotional Learning skills to converse about mental health and wellness with peers confidently. Skills consist of: i. Self-Awareness (Intrapersonal Focus) ii. Self-Management (Intrapersonal Focus) iii. Social Awareness (Interpersonal Focus) iv. Relationship Skills (Interpersonal Focus) v. Responsible Decision-Making (Inter and Intrapersonal) 4. The Student Wellness Leadership Training Program will focus on awareness and understanding of how chronic stress impacts communities of color and will provide experiential opportunities to practice wellness for themselves. 5. The curriculum must be relevant to Black and African youth. 6. The curriculum must include a copy of the LACDMH logo, the County Seal, the MHSA logo, and Black & African Heritage logo. 7. The curriculum and materials must be submitted to LACDMH for approval prior to implementation. All curriculum and materials belong to LACDMH. 8. Upon the request of LACDMH, the Facilitator must demonstrate that they fulfilled this deliverable by providing supportive documentation (e.g., receipts, contracts, attestations, etc.). 9. Payment is contingent upon completion of Deliverable activities (as listed above) and submission of all required paperwork.
