Specifications include, but are not limited to: The Asthma Management Academy (Academy) is a series of educational modules delivered to educators who primarily provide (or will provide) culturally and linguistically accessible, comprehensive asthma self-management and healthy homes education to patients or clients. The Contractor will deliver three 20-hour Academy sessions, track attendance, conduct evaluation, and meet regularly with CDPH. The Contractor will be responsible for carrying out activities and documenting deliverables to achieve the goals outlined in Exhibit A. Contractor shall perform the following services as specified in SOW, Attachment I: Conduct three 20-hour Academy training sessions to priority organizations, including federally qualified health centers, managed care plans, community clinics, non-profits, school-based health centers and community-based organizations conducting social services, asthma home visiting, or education services.
