Specifications include, but are not limited to: The Glenn County Health and Human Services Agency (HHSA), is seeking to solicit and award a contract to the highest qualified, experienced, and responsible Telephone Interviewing and Surveying Provider, hereinafter referred to as "Proposer," to assist the Department of Public Health with the Behavioral Risk Factor Surveillance System (BRFSS) and quantitative market research project. 1. Work with Glenn County to determine which year of CDC’s BRFSS core questions will be utilized dependent on county needs. 2. Include CDC’s required core components on topics such as the following (topics subject to change depending on which year of CDC BRFSS questions is selected): a. Health Status b. Healthy Days c. Health Care Access d. Exercise e. Hypertension Awareness f. Cholesterol Awareness g. Chronic Health Conditions h. Arthritis i. Demographics j. Disability k. Tobacco Use l. Alcohol Consumption m. Immunization n. Human Immunodeficiency Virus (H.I.V.)/Acquired Immunodeficiency Syndrome (AIDS) o. Fruits and Vegetables 3. Work with Glenn County to develop additional county-specific questions based off needs of Glenn County. a. Contractor will ensure county-specific questions undergo evaluation to meet standards for public use. b. Questionnaire length not to exceed 25 minutes per interview.
