A. Contractor shall provide a minimum of four (4) hours of direct patient services during each 8-hour workshop day and/or two (2) hours of direct patient services during each 4 hour workshop day. B. Contractor shall provide a twelve (12) week lesson plan for working with the patients upon commencement of the agreement term and update or revise the lesson plans when needed or requested. C. Contractor shall complete and submit prior to the end of scheduled services all required documentation, i.e. attendance records, sharps logs, weekly rosters, and provide staff with feedback for the purpose of progress reporting in a timely manner. D. Contractor shall provide support to special arts events, such as Arts in Mental Health Workshops, by assisting with performances and musical contributions. E. Contractor shall plan and provide a larger special arts event at least biannually to increase awareness of the Arts in Mental Health program, and assist with performances, musical contributions and/or display for a Metropolitan Art Exhibit (if applicable). F. Contractor shall contribute articles spotlighting dance workshops events for the seasonal Enhancement Services Newsletter upon DSH – Metropolitan Contract Manager request. G. Contractor shall attend the following training: All DSH Learn HAU requirements and an annual eight (8) hour Orientation Training. Contractor shall be required to attend training each contract year. The first contract year, Contractor shall attend Employee Orientation (8 hours) and TSI (Therapeutic Strategies and Intervention) Training (8 hours). The second contract year, Contractor shall attend Employee Orientation (8 hours) and TSI Training (4 hours). The Contractor shall be compensated at the rates provided in Exhibit B – Budget Detail & Payment Provisions. H. Contractor shall possess the physical capacity to transport needed supplies to and from workshop sites. I. All work shall be coordinated and approved by the DSH – Metropolitan Contract Manager. In the event additional work shall be required, the Contractor shall provide the DSH – Metropolitan Contract Manager with a written estimate for review and approval prior to commencing the work. J. Contractor shall not engage or make public statements and/or speeches, which could be construed to be official statements of Metropolitan and/or DSH without advanced approval from the Executive Director and the Chief Deputy Director of the DSH.
