Reentry FCM services shall be delivered to gang-involved youth exiting Probation camps, and their families, in a cycle of seven phases, as described below, and in accordance with the “GRYD Reentry FCM Policies & Procedures Handbook,” (Exhibit C). Reentry FCM services shall be provided over a period of six to nine months, upon release from custody. With the exception of Phase 1, which may last up to 90-120 days, each phase lasts approximately one month. ● Phase 1- Referral and Assessment: This phase is focused on pre-release services. Juveniles are referred to the designated Reentry Contractor for assessment of program eligibility. Reentry Contractor(s) meet with youth before release, develop a relationship with the youth inside camp, assist the family with their needs, plan a welcome home reception, and begin coordinating post-release services with the designated DPO. ● Phase 2- Building Agreements: Upon release from a camp, the Reentry Contractor shall help reorient the youth in the community, facilitate family engagement, and administer the Social Embeddedness Tool (SET) ● Phase 3- Strengthening Bonds: The youth begins working on a strengthbased Genogram, 2 obtaining necessary work documentation and identifying Tier 1 concerns ● Phase 4- Strategic Referrals: The youth begins receiving services for Tier 1 concerns, and participates in at least one Impact Event ● Phase 5- Celebrating Changes: A celebration is planned for youth and family to acknowledge the youth’s progress. Assigned Reentry Contractor staff member begins identifying Tier 2 concerns that the client would like to address. ● Phase 6- Next Level Agreements: The youth begins receiving services for Tier 2 concern(s), and continues working on the strength-based Genogram. The youth receives a SET reassessment 6 months after the enrollment OR at the completion of phase 6, whichever comes first.
