Selected vendor shall provide a Pharmacy Medication Therapy Management (MTM) hosted web- based platform system available for licensure to assist IEHP Pharmacists in the delivery of a CMS compliant MTM program. The System shall meet all CMS documentation and reporting. The selected vendor may also provide the services of pharmacists or other clinician who can perform CMR reviews. The Overall Project Objectives describes a Medicare Part D Pharmacy Medication Therapy Management (MTM) program, in which the selected vendor will implement designated responsibilities for IEHP. IEHP’s Medicare Part D members will receive services through the Program. The immediate goal is to identify a Vendor to provide licensure of a System for IEHP Pharmacists to identify and/or load members that meet MTM program eligibility requirements, deliver, track, document and report Comprehensive Medication Reviews (CMRs) and Targeted Medication Reviews (TMRs) and generate internal operational and CMS-required reports and generate CMS-required reports with the option to have the selected vendors’ Pharmacists assist with designated aspects of CMR and TMR delivery in accordance with CMS submission.
