HCA’s Employee Health Services (EHS) will coordinate with the successful Respondent(s) to schedule all appointments. EHS will also provide the successful Respondent(s) with the required forms, which are included in Attachment IV for reference, with the exception of the Federal Aviation Administration (FAA) form. Respondent(s) may submit a proposal for just the Physical Examination Services component of this solicitation, just the Occupational Medicine Physician Consultative Services component, or for each: the Physical Examination Services and the Occupational Medicine Physician Consultative Services components of this solicitation. Please see below for the requirements for each component. Please submit a proposal for each component as a standalone. The successful Respondent for both the Physical Examination Services and Occupational Medicine Physician Consultative Services, if applicable, shall meet all the requirements listed in both the Physical Examination Services section and the Occupational Medicine Physician Consultative Services section.
