Selected applicant(s) will be expected to deliver supported living services to adults eligible for regional center services with significant mental health needs. ELARC has identified at least six to eight initial individuals with intense challenges who want to live in their own homes and in home communities within the ELARC catchment area (Cities & Zip Codes for ELARC EXHIBIT A). Applicants are required to demonstrate their commitment to achieving the expected service outcomes consistent with the principles that are the guiding standards, fundamental values and the criteria for evaluating supported living services outcomes. Training should be sought from experts in the field. Local team (Crisis Team) approaches must be a component of problem solving. Consumers with significant mental health needs(s) and/or conditions will receive services and support in their own home and home community. One expected outcome is the prevention of psychiatric admissions and tenant stability. Supported Living Services (SLS) include personal supports and training that are tailored to meet the specific needs of people who want to live in their own homes. This array of services is available to anyone who desires facilitation, instruction, support and assistance to live in the home of his or her choice pursuant to the Welfare and Institutions Code 4689. Individuals “live in homes that they own or lease with support available as often and for as long as it is needed, when that is the preferred objective in the individual program plan.” The Eastern Los Angeles Regional Center (ELARC) is seeking qualified applicants to respond to this request for proposals. Applicants will be expected to demonstrate their knowledge, skills, abilities and competencies to assist consumers with autism and/or significant mental health needs in securing and/or maintaining their own homes and to provide consumers with the supports and training needed to live in their own homes. An individual receiving supported living services pays for her/his own living expenses (for example, including but not limited to rent, utilities, food and entertainment) out of their own income resources (such as SSI, work earnings or other personal resources). The regional center pays the vendor to provide supported living services. Individuals should also receive other kinds of eligible publicly funded services including but not limited to Medi-Cal, mental health services, vocational services, affordable housing/rental subsidies and In –Home Supportive Services (IHSS).
