Additional Bidding Instructions

The broker, in their response, should identify their approach and plan to provide as much support as possible for the remaining 2025 plan year and renewal years following. In addition, the broker should include the following in their response.

Cover Letter (no more than 2 pages)
Firm Overview

Full name and address of the firm with a brief description of the firm. Include a description of the following:

- Business organization
- Year established
- Location and address of the firm’s office that will provide the services requested
- A list of the firm’s primary services
- Team Experience

References

RFP reviewers may solicit references from some or all client contacts provided with previous experience examples above. Please include three references.

Compensation/Fees

Identify all costs including expenses to be charged for performing the services necessary to accomplish the contract's objectives. Please identify if fees are paid by the district or vendor. Costs for subcontractors are to be broken out separately.

Conflicts of Interest

Disclose any conflicts or perceived conflicts of interest. Identify what procedures your firm utilizes to identify and resolve conflicts of interest.

Insurance Requirements

A General Commercial Liability with at least $1 million policy and Workers Compensation at statutorily required limits. Refer to section five for more details.

Consultants must submit their statements in the format and order of the above-mentioned evaluation criteria. A cover letter and resumes or bios of team members should be included.
