Summary of Specifications:
This RFP is designed to allow the City of Littleton meet the requirements of Colorado Revised Statutes 8-43-404 (5)(a) as amended, which requires the employer or insurer to provide a list of at least two physicians or two corporate medical providers or at least one physician and one corporate medical provider, where available, from which list the injured employee may select the physician who attends such injured employee. Further, this Section requires the two designated providers be at two distinct locations without common ownership. Provide medical treatment and follow-up medical care to city employees injured or becoming ill in the course and scope of employment under the provisions of the Colorado Workers’ Compensation Act. In addition, services include: care from Level II Accredited Physicians; Employment physicals for a variety of job classes, including police, firefighter, Hazmat, DOT (CDL) and laboring; certified drug screening and breath alcohol testing; pulmonary functioning testing; vision and hearing testing; return-to-work medical evaluations and related testing.
