Summary of Specifications:
REQUEST FOR PROPOSAL for the Patronis Elementary School Multi-Use Path project will be received by the BOARD OF COUNTY COMMISSIONERS OF BAY COUNTY, FLORIDA at the Purchasing Department, 840 W. 11th Street, Panama City, Florida 32401 up until 1:30 PM (CST) on November 14, 2011. Proposals will be publicly opened at 3:30 PM (CST) on November 14, 2011.
Proposals shall be submitted in a sealed envelope, plainly marked with Proposer’s name, address, date and time of bid opening, and RFP number, Patronis Elementary School Multi-Use Path Design and Construction firm’s proof of payment of Bay County taxes must be included in the Proposal package and may be obtained by calling the Bay county Tax Collector’s office at (850) 248 8501.
Pre-qualification: Proposal packages must include proof of FDOT qualification (current FDOT Notice or certificate of Qualification) in the following categories: All design firms, to include construction firms with in-house design teams and sub-consultants (surveying, geotechnical, etc.) shall be qualified under Rule Chapter 14-75, Florida Administrative Code, in the following types of work: Group 3, Type 3.1 Minor Highway Design, Group 8 Type 8.2 Design, Right of Way and Construction Surveying, Group 9, Types 9.1 Soil Exploration, 9.2 geotechnical Classification Lab Testing and 9.3 Highway Materials Testing. The prime construction contractor and all subcontractors shall be qualified under Rule Chapter 14-22, Florida Administrative Code, in the following class of work: Grading, Drainage, and Specialty Classes of Work: Grassing, Seeding and Sodding, and Pavement Markings. If asphaltic concrete is used the paving contractor shall also be qualified in Flexible Paving, Hot Plant-Mix Bituminous. The asphaltic concrete shall be produced in an FDOT approved concrete production plant located in Bay Count with an FDOT approved asphaltic concrete laboratory on-site.
