RFP-14-03 - Group Life Insurance Plan

Summary of Specifications:
The School District of Okaloosa County, Florida extends an invitation to all interested insurance companies, agencies, and agents Ulicensed in the State of Florida to submit proposals for providing a Group Term Life plan to approximately 5,006 eligible participants including active employees and retirees. This proposal, as well as the rates, will be guaranteed for three (3) years with an effective date of January 1, 2014. Continuation of the coverage is contingent upon annual review and agreement between the selected provider and the School District of Okaloosa County. Any renewal of the agreement will be subject to satisfactory servicing of the plan, satisfactory participation in the plan and acceptable continuation terms. The attached Life Certificate (2011 CERT – LIFE.pdf) is the plan now available.
