The City is interested in proposals for plan designs that most closely match the City’s current plan designs for medical and prescription coverage: 1. Florida Blue- HMO BlueCare HSA 120/121 2. Florida Blue- HMO BlueCare 47 3. Florida Blue- HMO BlueCare 67 4. Florida Blue- PPO Blue Choice 05771 The City currently has a Pro-Share, participating insurance contract with Florida Blue. Please see the Exposure and Loss Data Section for more information. Please state if you have the ability or are offering another type of contract and under what circumstances. The prescription benefit should be proposed as similar as possible to the current co-pays by plan, shown in the Exposure and Loss Data Section of the RFP. This RFP includes a Benefits Match-Up (A and B) Item 4, outlining the current benefits and asking proposers to respond “Match” or provide details regarding the benefits offered for the plan proposed.
