This is a Sources Sought Notice only. This notice is for market research purposes to identify small businesses capable of performing/providing a requirement. This is not a solicitation. This notice does not obligate the Government to award a contract. A solicitation will not receive a response. The Government will not pay nor reimburse any costs of preparing this response. The notice is to gain knowledge of potential qualified businesses and their Small Business Administration (SBA) size classification in relation to North American Industry Classification System (NAICS) 621610, Home Health Care Services with an associated size standard of $19.0 million dollars. The Department of Veterans Affairs (VA), Veteran Integrated Service Network (VISN) 8 South Contracting Team is seeking interested sources to provide the following services of pickup, setup, disassembly, repair, routine preventive/service/refurbishment maintenance, site assessment, disposal, storage and delivery service of Durable Medical Equipment (DME) and supplies to all beneficiaries serviced by VISN 8s VA West Palm Beach and Miami VA Medical Centers (VAMC) and their associated outpatient clinics. The contractor awarded this program requirement will service that entire multi-area and VA patients. The contract period will be a base of twelve months and four twelve month options. The contractor shall provide all facilities, labor, parts, equipment, transportation, fuel, and supervision necessary to provide storage, delivery, set up, repairs, in-house and onsite instruction, and/or pick-up of durable medical equipment for VA beneficiaries of the VAMC. Only those firms who respond to this announcement by submitting the following information will be used in determining whether to set aside or compete this requirement. Please provide all of the following: 1) A capability statement expressing interest in this requirement, describing your company and its capability to complete this work. Include a statement detailing any special qualifications and certifications, applicable to the scope of this project, held by your firm and/or in-house personnel. The capability statement must directly address durable medical equipment and service to patients over a multi-state area with the bulk of the catchment area resided in the state of Florida. 2) A statement or list of your firms current or past technical experience similar to or the same as the summary scope of work for this requirement. Include a brief description of the scope and schedule for each similar requirement. 3) A statement or list of your firms current or past performance similar to or the same as the summary scope of work. Include a point-of-contact name, phone number and email address. 4) A statement of your firm s business size and type (HUBZone, Service Disabled Veteran Owned Small Business, 8(a), Woman-Owned, Small Business, etc) utilizing the NAICS code and size standard noted above.
