1. General Requirements a. Contractor must possess a Class I, II, and III Certificate of Authority (COA) issued by the State Fire Marshall’s office and maintain the COA for the duration of the contract term. A copy of the COA shall be made available to the District to maintain on file. b. All services shall be performed by qualified technicians trained on the specific equipment being maintained. Contractor shall provide proof of training upon request. c. In addition to the activities listed in this document, equipment model-specific maintenance procedures specified in the manufacturer’s service manual shall be performed at the suggested service intervals. d. Contractor shall provide and install the manufacturer’s maintenance kit specified for the equipment model being serviced. Kits shall include all ignitors, flame rods, gaskets, filters, etc. necessary to complete the service as specified by the manufacture. e. Contractor shall provide a written report each time maintenance is performed on a piece of equipment. The report shall list and confirm all maintenance steps performed, the results of all required safety checks, a list of all deficiencies found, and a summary of all findings and recommendations. f. Contractor shall coordinate schedules with the assigned jurisdictional inspector for all internal inspections. g. Contractor will maintain an accurate and current inventory, in electronic format, of all equipment under contract and provide to the District. At a minimum, the inventory will include the following attributes: i. Asset name ii. Building/Location iii. Equipment Type (Steam Boiler, HW Boiler, Water Heater, iv. Manufacturer v. Model Number vi. Serial Number vii. Use (Heat, Hot Water, Heat and Hot Water, Process) viii. Fuel Type ix. Capacity stated in BHP or BTU/HR x. Year Manufactured xi. National Board Number xii. State Jurisdiction Number 2. Frequency a. Preventative maintenance shall be performed on all in-scope equipment twice per year. i. Semi-annual preventative maintenance services shall be performed as close as possible to the start of the heating season to help ensure equipment is safe and operating reliably. Maintenance activities and test results shall be logged, and deficiencies promptly reported to customer. ii. Annual preventative maintenance services shall be performed as close as possible to the end of the heating season. Maintenance activities and test results shall be logged, and deficiencies promptly reported to the District. 3. Semi-Annual Preventative Maintenance Scope of Work All Equipment The following scope of work shall apply to all equipment under contract, regardless of equipment type, size, or manufacturer. a. Operational testing i. Test fire unit multiple times to ensure reliable operation. Check for fault codes ii. Test all operational capabilities of the unit iii. Check error logs for recent issues iv. Check all gauges, monitors and indicators v. Check operating limits and adjust as needed b. Safety device testing i. All safety controls as defined by ASME CSD-1 shall be tested in accordance with code requirements, including: 1. Low Water switches 2. Flow switches 3. High pressure or high temp limits 4. Emergency stop switches 5. Air proving switches 6. Low and high gas pressure switches c. Burner service i. Inspect pilot and adjust ignition electrode ii. Check flame strength signal iii. Check flue gas readings with combustion analyzer and tune burner for optimum efficiency. Provide print out. iv. Check for and remove any obstruction to the flow of combustion air to burner d. Fire side maintenance i. Check fire side gasket material for CO leaks e. Water side maintenance i. Check pressure vessel for leaks ii. Check water side gaskets for leaks f. Pumps and feed water systems i. Test proper operation of boiler and system loop pumps
