Vendor shall provide One (1) Standup Rider Electric Forklift, Narrow Aisle Lift: 4,000 lb. Basic Capacity, 36-volt, Fork Height Min = 18 feet. Specifications will be like the Toyota HPT50 or Clark ESX20. The rental must include all maintenance, parts and materials required for the forklift operation, including the battery charger. The Vendor shall provide the Correctional Center with the name and telephone number of a responsible representative for emergencies or any other special requirements. If deemed an emergency call-out by the facility Chief Engineer, Vendor must respond to the issue within 24 hours of notice. Discrepancies in, or omissions from, this instrument should be immediately called to the attention of the Warden of Decatur Correctional Center. No Third-Party Agreements will be accepted. The terms and conditions shall be accepted by the vendor as outlined in the State of Illinois Standard Terms and Conditions section. When a brand name or equal specification is used in a solicitation, the solicitation shall contain explanatory language that the use of a brand name is for the purpose of describing the standard of quality, performance, and characteristics desired and is not intended to limit or restrict competition. "Or equal" submissions will not be rejected because of minor differences in design, construction or features that do not affect the suitability of the product for its intended use. Burden of proof that the product is equal is on the bidder. For procurements conducted in BidBuy, the State may include in this contract the BidBuy Purchase Order as it contains the agreed Supplies and/or Services.
