Specifications include, but are not limited to: It is the intent of the Lake County Health Department and Community Health Center (hereinafter referred to as "theHealth Department") to secure services from a qualified contractor for the provision of Radiology and UltrasoundServices for patients of the Lake County Health Department and Community Health Centers. The services shall beon a referral basis with patients referred for services from Lake County Health Centers located in Waukegan at theBelvidere Health Center and the Grand Avenue Health Center, North Chicago Health Center in North Chicago,Northeast Health Center in Zion, MidLakes Health Center in Round Lake Beach and the North Shore Health Centerin Highland Park. The successful Contractors location(s) shall be easily and conveniently accessible through thevarious public transportation options in Lake County. It is estimated the Health Department will referapproximately 700 to 1200 X-Rays and 1000 to 1200 Ultrasounds annually.
