STATEMENT OF WORK Provide Milk/Dairy Delivery Contractor shall provide the estimated Milk/Dairy Delivery requirements as described in the schedule below to the Location: US Department of Veterans Affairs, Richard R Roudebush Medical Center 1481 West 10th Street Indianapolis, IN 46202; in accordance with the specifications contained herein. The Government anticipates award of an Economic Price Adjustment contract (FAR 52.212.-2) for milk and dairy products. This contract will be effective for a base plus 4-year period effective 10.01.2024 through 09.31.2028. The Government does not guarantee the estimated yearly quantity. Any proposed alternate item shall meet the requirements of the Government specification as cited in the Index of Federal Specifications/Standards (FED) and or commercial Item Description (CID). The awarded contract shall constitute the entire agreement including all terms and conditions applicable to the contract. The Government, at its discretion, may exercise up to four (4) option periods of one (1) year each. The contractor shall perform no services after sept, 30th of each year until the Contracting Officer authorizes such services in writing. a. Delivery Location: 1481 west 10th street Indianapolis, Indiana, 46202 b. Delivery Hours: 7am to 9am. c. Delivery Frequency: two (2) days per week (Monday, Friday) d. Delivery expectation:10.01.2024 through 09.31.2028 Daily deliveries will be at or about the same time each day and within the established delivery hours. If a delivery date falls on a federal holiday, the vendor must establish an alternative delivery date and inform the VA Hospital of the new date(s). Deliveries must be delivered in a refrigerated truck and delivered below 41 degrees Fahrenheit. The Federal Health Care Center has the right to refuse the delivery if products are above 41 degrees Fahrenheit. Contractor-owned delivery trays/racks and wheeled carts, etc. shall be in clean condition at delivery time; if they are not, contractor personnel will be required to transfer the milk delivery to government owned holding cart. iv. The government will hold contractor-owned crates, wheeled carts, utilized in delivery pick up by the contractor for reuse. Contractor is required to pick up empty, contractor owned delivery crates, and wheeled carts, etc. from previous deliveries, upon each delivery of new product. Empty equipment shall not be allowed to accumulate at the VA facility in storage rooms, warehouses, loading docks, or any other areas on the VA property. United States Department of Agriculture (USDA) standards and guidelines: All current USDA standards and guidelines must be met throughout the entire production, packaging, labeling and delivery process. Product Freshness The expiration date stamped on each container of fluid milk must be a minimum of ten (10) days postdate of delivery. The expiration date of non-fluid milk/dairy products must be a minimum of fourteen (14) days postdate of delivery. Orders Authorized personnel from Nutrition and Food Service will place all delivery orders. Orders should be placed by phone and/or online ordering system. Order is based on facility needs and amounts will increase or decrease in conjunction with demand. All payments must be submitted via Electronic Funds Transfer. Invoices must be submitted to the Government monthly in the arrears, by direct bill through TUNGSTEN (tungsten-network.com) . Failure to Deliver If the contractor fails to deliver the requested item(s) or acceptable substitute, or to deliver during the required time, the Government may purchase the item from another source(s). GENERAL REQUIREMENTS C.1.1. The contractor shall contact the Contracting Officer on all matters pertaining to administration. Only the contracting Officer is authorized to make commitments to issue changes which will affect the price, quantity, quality, or delivery terms of this contract. C.2.0. ALTERNATE ITEMS, PACKAGING, AND PACKING: C.2.1. Offerors may propose alternate items, packaging, or packing. Offerors shall clearly indicate, in their offer, the quantity, packaging size, unit or other different features upon which the offer is submitted. Evaluation of the alternate item will be made against a common denominator (i.e.; weight, package quantity, etc.). Offerors are reminded that the alternate item must meet all the requirements of the Government specifications. C.2.2. Availability of requirements documents cited in the solicitation: a) The GSA Index of Federal Specifications (FED), Standards and Commercial Item Descriptions (CID), FPMR Part 101-29, and copies of specifications, standards, and commercial item descriptions cited in this solicitation may be obtained for a fee by submitting a request to: GSA Federal Supply Service (Address) b) If the General Services Administration, Department of Agriculture, or Department of Veterans Affairs issued this solicitation, a single copy of specifications, standards, and commercial item descriptions cited in this solicitation may be obtained free of charge by submitting a request to the addressee in paragraph C2.2.(a) of this provision. Additional copies will be issued for a fee. C.3.0. PRODUCT QUALITY AND FRESHNESS: C.3.1. All Milk/Dairy Contract goods shall be delivered on a Monday and Friday from 7am-9am. Each order must comply with product specifications by the Department of Agriculture standards and arrive in a timely manner- between 7-9am, no later than 1 hours past scheduled time window in the case of inclement weather. C.3.2. All products furnished under this contract shall be processed and packed under sanitary conditions in strict accordance with guidelines provided by the U.S. Food and Drug Administration and the U.S. Department of Agriculture, using the best commercial practices that are standard for the industry. All containers shall be clean, sound, and securely covered or sealed to provide adequate protection from dirt, filth, or contamination. C.3.3. Unless otherwise specified in the item specification or solicitation, marking shall be in accordance with best commercial practice. C.3.4. Any commercial or additional labeling, as specified, which complies with the U.S. Food, Drug, and Cosmetic Act, and regulations promulgated there under, is acceptable. C.3.5. Products manufactured to metric dimensions will be considered on an equal basis with those manufactured using inch-pound units, providing they fall within the tolerances specified using conversion tables contained in the latest revision of Federal Standard No. 376. If a product is manufactured to metric dimensions and those dimensions exceed the tolerances specified in the inch-pound units, a request should be made to the Contracting Officer to determine if the product is acceptable. The COR, in accordance with the Contracting Officer, will accept or reject the product. C.3.6. Additional packaging, packing, and/or marking requirements can be found in the Addendum to 52.212-4, Terms and Conditions Commercial Items. C.4.0. DELIVERIES OR PERFORMANCE: C.4.1 The Contracting Officer shall authorize Medical Center personnel to place delivery orders for dairy products listed in the schedule of pricing. C.4.2 Orders shall be placed no less than 1 workday prior to required delivery date, upon delivery at the receiving location, items ordered shall be of acceptable quality, quantity and appearance as outlined in US Food and Drug Administration and the US Department of Agriculture, best commercial practices that are standard for the industry. C.4.3. Deliveries shall be made between 7:00 a.m. and 9:00 a.m., local time, on Monday and Friday, as requested by the COR or his/her designee, excluding Sundays and designated Federal Holidays. There can be a 1-hour delay on deliveries in case of in climate weather.
