Specifications include, but are not limited to: provide professional occupational therapy services in large longterm care facility, as needed, based on weekly physicians’ orders. The Service Provider will perform work involving the assessment of motor function, and/or person-social development and treatment of residents at the state facility. Professional services will be provided to residents on all levels of care as the need is identified by the primary provider. IVH will provide all supplies, equipment, materials and property for Occupational Therapy service. The Service Provider will perform occupational therapy assessments and evaluations using appropriate tests, skilled, professional observations and supplementary information. They will also enter data into the facilities electronic medical record system (ADL) for services performed for each resident. Contractor will provide Occupational Therapy Services on an average of 24-28 hours per week but not less than 8 hours per month. Pricing should be inclusive of all travel costs. Services will also include providing monthly supervision hours, oversight and direction for one Certified Occupational Therapy Assistant. Must be skilled with elderly, chronically ill, and cognitively and physically challenged patients. The Service Provider must ensure that suitable measures are taken to assure the confidentiality of IVH and its residents at all times and abides by a professional code of ethics.
