Specifications include, but are not limited to: Iowa Veterans Home is seeking Iowa Licensed Occupational Therapy Services for our residents in a long term care facility setting based on weekly Provider orders. Services would include an Occupational Therapist for a minimum of 16 hrs/month split up into at least 4 hours weekly care and a Certified Occupational Therapy Aide to assist with facility needs/ongoing therapy under the supervision of the Occupational Therapist for a minimum of 20 hrs/week. These services can also be combined and provided by one Occupational Therapist. Once this bid is complete all OT services will be under this contract. Respondent will be responsible for performing work involving the assessment of motor function, and/or person-social development and treatment of residents at the state facility. Professional services will be provided to residents on all levels of care as the need is identified by the primary provider. They will also enter data into the facilities electronic medical record system (ADL) for services performed for each resident. IVH will provide all supplies, equipment, materials and property for Occupational Therapy service.
