Please provide a quote for onsite flu shots, Covid-19 vaccine, Covid-19 boosters, and shingles shots clinics. Clinics to be held September 15th through the end of October 2024. 1.) Clinics will be held at 12-15 locations on the Capitol Complex Campus, Ankeny and Ames to accommodate approximately 300 employees. 2.) Please provide 9 or 10 different dates available for scheduling the clinics. 3.) Contractor must be able to bill the employee's insurance and also accept payment from employees at the time of the shot/mist. Please provide an explanation of the options that are available for payment. 4.) Contractor must accept Tricare insurance. 5.) Contractor must work with the site coordinators to schedule clinics at each location. 6.) Please provide pricing for flu shots, Covid-19 vaccine, Covid-19 boosters, and shingles shots. (Please list the Covid19 vaccine manufacturers) 7.) Please provide a sample flyer template that can be used for promotion of the clinics.
