Contractor to provide all materials, labor, mobilization, traffic control, and equipment necessary for the repair of the concrete head wall on the southeast corner of the south bound bridge on I-380 at 50.6 mm. This work can be done day or night. Location: South bound bridge on I-380 at 50.6 mm. Requirements: -All material must be new and not used. -Contractor must remove all remaining damaged concrete head wall from site. Material becomes property of contractor to dispose of off the project site. -Contractor must be registered with the Iowa Division of Labor. - Provide a current Traffic Control Technician Certificate per Standard Specification section C.1. of Section 2528 Traffic Control and must follow all applicable provisions of section 2528 when performing traffic control. -Contractor to provide certificate of insurance per the standard terms and conditions found under the attachments tab. -The Contractor must provide any grade work necessary to return the location back to the original grade prior to project start.
