Off-Site, Retail Services A. Contractor shall make available convenient, statewide commercial fueling locations. All commercially available fuel types should be available for purchase with fuel card. These sites must be capable of electronically processing fuel transactions that will be integrated into contractor’s weekly fuel report B. Assumption Parish Police Jury will be supplied with control access cards providing fueling and driver identification, security code identification number, fuel type and card loss control (upon 24-hour notice), at no cost to the Assumption Parish Police Jury. Required Reports A. Unleaded and Diesel Fuels i) For unleaded and diesel fuels, Contractor will provide via email to the Assumption Parish Police Jury and through online access a weekly report by vehicle to include, but not be limited to, the following: 1) Vehicle number and description in a manner that identify pin used by name, cost center and agency to assist in accounting measures. Vehicle number shall contain a minimum of six (6) characters - right justified, blank filled on electronic format. 2) Date and time of fueling. 3) Location of fueling. 4) Total number of gallons dispensed into vehicle. 5) Type of fuel purchased. 6) Base price and extended total of fueling. 7) Taxes paid per gallon (give name/breakdown of type of tax charged). 8) Odometer (in the case of off-road equipment the entry for mileage shall represent hour meter readings.) 9) Summary totals of miles driven, average mile per gallon, quantity and cost of fuel purchased for this reporting period for each vehicle. 10) A report providing total number of gallons by product type. 11) A reference table for all fueling locations. ii) Contractor will provide Assumption Parish Police Jury with a real time online account management system allowing Assumption Parish Police Jury to: lock cards, reinstate cards, view pin number information, modify authorization parameters of individual vehicles or drivers and view real time transition information.
