Program Planning A. The Applicant shall design a new program or add to an existing program within the organization. B. Overall program design shall include comprehensive plans to: a. Identify and address the needs of diverse populations. b. Comply with and meet all deadlines for contract deliverables. C. Program design shall include the development or adoption of evidence-based intake evaluation tools and action plan templates. Applicants should discuss which tool(s) will be used or how those decisions will be made upon award in their application. D. Program design shall include the development of protocols and workflows for: intake, consent, and other administrative components; development of and feedback on individual action plans; program satisfaction survey forms; and plans to build or enhance community relationships, either through the development of new materials or adaptation of existing materials, within the first 6 months of the award.
