The Maryland Department of Health, Office of Children and Youth with Specific Health Care Needs (“OCYSHCN”) intends to develop uniform care navigation standards and apply them to a statewide care navigation program. This project will standardize tools and metrics, allowing for uniform data collection and more impactful analysis and understanding of CSHCN within the state. This planning grant will serve to inform the development of the program by presenting recommendations, background information, and options for OCYSHCN’s use and testing. This grant supports 18 months of planning and consultation with a resulting deliverable of a program plan for a statewide system of care navigation. It is the State’s intention to obtain services, as specified in this request for applications, from an agreement between the selected applicant and the State. The anticipated duration of services to be provided under this agreement is March 1, 2025 to June 30, 2026.
