Specifications include, but are not limited to: opening 1-3 homes in the Metro Region or neighboring communities for 4-5 individuals at each site. The individuals will be leaving special education services or have left school over the last two years. Two of the sites will support individuals with intensive behavioral needs including but not limited to aggression, self-injury, property destruction and elopement. The individuals in these sites will require the availability of strong clinical services, including on-site support. An accessible site is preferred. One home will support individuals with a high level of self-care needs. This site will need to be fully accessible. Medical oversight will be needed. The Region will work closely with the chosen Bidders to refer individuals appropriate for placement in these homes. It is the intent of the Region to provide reimbursement based on staffing patterns and clinical supports negotiated with the vendor based on the needs of the individuals identified for each home
