25-0024 - Ambulance Billing & Collection

1. The proposal must detail the consultant’s qualifications, experience and expertise. Proposal evaluation will include an examination of the consultant’s qualifications, experience and expertise in providing an ambulance billing & collection service.
   1. Consultant shall provide a brief history of his/her experience. 
   2. Consultant shall detail his/her experience with providing billing and collection services during the past three (3) years for organizations similar to the Town of West Springfield.  
   3. A list of specific qualifications the consultant has in supplying the services listed in this proposal, including any professional designations and affiliations, certifications or licenses. 
   4. Consultant must submit the names and resumes and level of participation of staff that will be assigned to this engagement and their respective experience in these types of engagements. 
   5. Provide a reference list of all current and/or former clients for the past three years that may be contacted regarding consultant’s performance.  List shall include client contact name, agency, address and phone number. 

2. The consultant must confirm their understanding of the RFP.  
3. Explain the consultant’s administrative process will ensure that appropriate levels of attention are given and that the work is properly performed.

The price proposal should contain an amount for the completion of the scope of services, using the enclosed “Price Proposal”.  

No reference to costs may be included in other sections of your proposal.
