Specifications include, but are not limited to: This RFP is to procure case management, housing assistance, mentorship, and pre-college education services for youth in transition from foster care or a delinquency placement to independent living that have a closed MDHHS case. The purpose of the Youth in Transition (YIT) Program is to provide independent living services to eligible youth as defined by the John H. Chafee Independent Living Act of 1999. Services must be coordinated with other federal and state programs for youth including, but not limited to youth homeless/housing programs, programs for disabled youth, school to work programs, employment programs, sexual health, pregnancy prevention and abstinence programs and health care services before utilization of YIT funds. All other available resources for independent living services must be exhausted prior to utilizing YIT funds.
