The Veterinary Services providers “Contractors” will provide services and medications as approved by the City. Perform examination, diagnostics, and treatment as indicated and approved by agent. Promptly provide notification of when treatment has been completed and pick up is needed. Dispense appropriate medication with written instructions with animal at time of discharge. All services should ensure animals are cared for in accordance with local ordinances and state laws. All staff members shall be appropriately licensed as required by the State of Michigan. The responsibility for the care of the animals will be the sole responsibility of the Proposer from the time they are dropped off until picked up. The Contractor should provide an invoice for services identifying the City’s animal identification number and type of medical procedures performed. The contractor shall provide invoices/estimates for all visits which must be approved by the Fire Investigation Chief.
