Assess and document the need for placement of a child in a QRTP. The Assessor must: Review all documentation provided by the SPD, JJAU, or AGAO. Work in conjunction with the child’s permanency team to obtain information regarding the child’s circumstances and level of functioning and report the findings. Permanency team members include: Legal and/or biological family members. Relative and fictive kin of the child. Professionals who have treated or are a resource to the child and/or family, including but not limited to teachers, medical and mental health providers, and clergy. In the case of a child who is age 14 or older, the permanency team must include members that are selected by the child. Review the MichiCANS completed by MDHHS or Contractor. If applicable, the Contractor must also review the Michigan Juvenile Justice Assessment System risk assessment tool completed by MDHHS. Complete a written Independent Assessment report using the template provided by MDHHS describing finding, the child’s level of functioning, needs and strengths, short and long terms mental health and behavioral health goals, and reasons for recommendation for treatment and placement setting. The report must include: Identifying information for the child. Permanency team member information including the name of who was contacted, contact method, and information obtained from the contact including but not limited to placement preference.
