Dakota County Public Health Department Family Home Visiting Program is seeking partnership with a community-based organization to expand access to family home visiting services for the East African perinatal families (pregnancy through first year of life of child). The goals of family home visiting include improve and promote parental and child health, enhance positive parenting practices, prevent child abuse and neglect, reduce intimate partner violence, promote children’s development and readiness to participate in school and connect families to needed community resources and supports. The purpose of this request for proposal is to seek partnership and technical assistance from a community-based partner that is familiar with perinatal East African families. The partner will be a subject matter expert on the defined population and provide training for staff in the public health department. The partner will also work with Dakota County Public Health to utilize a methodology to seek community input on strengths and challenges of perinatal East African Families. The community partner will help assist with program development including choosing or developing curricula, educational materials and participant supplies. The community partner will also help plan, coordinate and implement outreach events with goal to enroll families into a family home visiting program and family group activities and connect East African families to other community resources. The partner will also help develop and disseminate program evaluation to participants and participate in continuous quality improvement of this program.
