Specifications include, but are not limited to: One RFP proposal submission may include one or more of the following three service positions: (1) School-Based Care Coordinators - School-based care coordinators are responsible for the day-to-day oversight and management of assisting enrolled LEAP youth in achieving success in reaching educational goals. The school-based care coordinator provides progress reports and has regular contact with the youth, parent(s)/guardian(s), educational team, and Hennepin County staff working with the youth. Schoolbased care coordinators will assist with problem solving youth participation/attendance, behavioral concerns and coordinating other supportive mental/chemical health referrals and services. The school-based care coordinator must have a valid driver’s license to transport youth as needed depending on sites rules. School-based care coordination requires a 1.0 Full Time Equivalency (FTE) coordinator on-site at each school site. The school-based care coordinator position requires staff with a minimum of a bachelor’s degree in human services, education, psychology, social work or other related field, and experience working with the priority population. Examples of services provided by the school-based care coordinator role include but are not limited to: • Obtain parent(s)/guardian(s) signature on the Release of Information (ROI) forms; • Provide care coordination for caseloads of 20 youth per school-based care coordinator referred to Plus programming; • Provide advice, guidance and support to youth, parent(s)/guardian(s) and other identified support teams; • Connect youth to resources for housing, food, and basic needs; • Crisis planning, as needed, with youth and family; • Write weekly case notes for each youth on caseload and document in a data system; • Staff attendance at monthly meetings and trainings; • Help provide transportation of youth to school, as needed; • Support youth with expungement of criminal records; and • Other job activities as determined by site. This RFP anticipates funding between eight and ten school-based care coordinator positions. (2) Chemical Health Specialist - Chemical health specialists receive referrals from Plus school-based care coordinators, school staff or self-referrals from youth. Chemical health specialists provide on-site chemical health screening and education services to referred youth who are impacted by substance use/abuse, which may include family concerns, stress management, self-esteem and pro-social decision making. Chemical health specialists will provide advice, guidance and support to youth, parent(s)/guardian(s), and Hennepin County staff around chemical health use/abuse. Chemical health specialists also complete required documentation of weekly service provided. (Services provided by the chemical health specialist within the Plus program are not considered chemical health treatment.) This is a 1.0 FTEposition and must be provided by a licensed drug and alcohol counselor (LADC) who is experienced working with the priority population. This position may travel to multiple school locations. This RFP anticipates funding two chemical health specialist positions.
