Specifications include, but are not limited to: The Department of Health and Senior Services (DHSS), Center of Health Innovations Network (CHIN) requires a contract for a Chronic Kidney Disease (CKD) Learning Collaborative in Missouri that supports Federally Qualified Health Clinics (FQHCs) with engagement in a CKD population health model. The Centers for Disease Control and Prevention (CDC) 1817 grant supports the implementation and evaluation of evidence-based strategies to prevent and control high-burden populations. The program includes a strategy to increase the use of clinical decision support within electronic health records (EHRs) to promote early detection of CKD in people with diabetes. DHSS will allocate funding from the CDC to the National Kidney Foundation (NKF) for these services. NKF has a statewide focus, is capable of managing the required services on a statewide basis, and is singularly focused to rapidly and effectively fulfill the strategy specific to improving CKD. NKF is the sole, feasible source for the required services in Missouri. As a result, awarding a contract to NKF represents a proprietary requirement that meets the criteria as a single feasible source under RSMo 34.044, and the state is pursuing this acquisition as a Single Feasible Source.
