Bid Number 4746 - Wyandotte Municipal Svcs Concrete Foundations and Sitework Construction Services

Wyandotte Municipal Services power plant (WMS-PP) located in Wyandotte, Michigan, requests services for concrete foundations and site work construction for the Wyandotte Municipal Services (WMS) in preparation for a new Auxillary boiler house.  

The proposal MUST be submitted with the following: 
1. A written proposal outlined as exhibited in Section 1.4 
2. Bidder Qualifications Questionnaire – completed (Appendix A) 
3. Firm Cost Breakdown (Appendix B) 
4. List of Exceptions to Terms and Conditions (Appendix C), Contract Agreement (Appendix F), or Contract Change Form (Appendix G). 
5. Signed Collusion Affidavit (Appendix D) 
6. Signed Bond Proposal (Appendix E) (with Bid Bond as stated above) 
7. Final Detailed Level 3 Project Schedule with Lead Times 

Note the contract agreement form in Appendix F and contract change form in Appendix G is for Reference Only and does not need to be completed for proposal submission. If the submittal is incomplete or not received at the address above before the time listed, WMS reserves the right to disqualify the bid immediately.  

Bidder shall fill out the Questionnaire completely and must address the following: 
• Bidder has supplied the service described in these specifications for a minimum of 10 years. 
• Bidder must supply three (3) client contacts in which Bidder is contracted for auger cast pile foundations. 
• The EMR safety rating is less than 1.0 for the current year
