Summary of Specifications:
The County of Ottawa (County) is seeking proposals from qualified bidders to conduct a survey of behavioral risk factors in Ottawa County similar to the Center for Disease Control and Prevention’s BRFSS survey. The purpose of this survey is collect county specific data on health practices and risk behaviors that are linked to chronic diseases, injuries and preventable illness in the adult population. This information will be used to assess the community needs, evaluate programs and plan for the future.
