37-21-037 - Care Management Organization Services

The County of Wayne is requesting proposals for providing Care Management services. As a lead agency or Care Management Organization (CMO), the vendor(s) will have unconditional responsibility for active and newly adjudicated juvenile cases within a cluster of contiguous zip codes (Appendix E) as determined by Wayne County. The contracted provider(s) has the responsibility to ensure core juvenile services are delivered: case management, service planning, Balanced and Restorative Justice (BARJ), court services, community-based services, residential placement and establish a network for treatment services.
