Complete and submit the SFQ-Quote Form, Appendix A - Price Sheet, Ethics Form, and the Form W-9 with your Bid Response. You may include brochures and/or specification lists of the products if you have them available, but it is not required.
The SFQ-Quote Form and the Appendix-A Price Sheet are mandatory documents. Failure to submit these documents with your bid response will deem the bid response non-responsive and no further evaluation or consideration will be give to the bid response.
Please do not wait until the last minute to submit your bid response, just in case there are technical issues. Late bid responses are not accepted by BidNet Direct, nor will they be accepted by Wayne County.
