18-130 - CASE MANAGEMENT AND REFERRAL SERVICES FOR THE GENESEE COUNTY OFFICE OF SR SRVC

Services are to be delivered to seniors aged 60 years and older, whose residences are located at scattered sites throughout the entirety of Genesee County's 639 square miles of land area. Services shall be provided that are inclusive of initial and/or pre-screening intake assessments, comprehensive assessments, coordinating clients' needs and making referrals to/from needed community services on behalf of the client from a one time basis to ongoing, and long term coordination, reassessments, multiple referrals and follow through.  If a language barrier exists for the senior client, the Service Provider shall utilize appropriate interpretive services for the non-English speaking seniors.
