Summary of Specifications:
Contractors with proficiency to design and construct/renovate an 8 x 28 foot space into 3 exam rooms. 1. Provide cost estimate for the design and construct/renovate an 8 x 28 foot space currently used as a lab, into 3 exam rooms. Each exam room would require the following: a. Faucet , Sink and Cabinet b. Counter/Work Station Area All of the above must be included within the Construction Services Proposal. All proposals must include: 1. Evidence of contractor’s qualifications to provide the above services 2. Three (3) client references 3. Proof of Insurance and W9 4.
