Summary of Specifications:
Contractors with proficiency to design and construct a Second Floor Addition over our Wellness Center. Must provide a cost estimate for the design and construction of a second floor addition to Wellness Center that would include the a Receptionist Area, Public & Employee Restrooms, Medical Lab, Exam Rooms, Provider Room to accommodate up to six individuals, Janitor Closet and Storage Room. All of the above must be included within the Construction Services Proposal. All proposals must include:

1. Evidence of contractor’s qualifications to provide the above services 2. Three (3) client references 3. Proof of Insurance and W9 4. Total cost to Muskegon Family Care
