1.2       Provide the following information/documents.
A. Provide a brief narrative of your company’s experience to include experience with government contracts.
B. Provide information on the following on your ability to provide the following:

Realtime electronic reporting method
Able to pull data/prescription information from NPI number.
Access to Scriptfiles
Works with independent pharmacies
Ability to pull monthly reports based on claims.
Ability to provide quarterly reports.

C. Provide 3 references. Contractors should include the contact’s name, phone number, and email address of these references.
D. Provide resumes for staff to be assigned.
