Specifications include, but are not limited to: 1. PLAN DOCUMENTS Draft plan documents, summary plan descriptions, plan booklets, benefit provisions, identification cards, certificates of credible coverage as required under HIPAA, changes in plan benefits, and any other material needed to properly administer claims in accordance with the provisions of the plan, collective bargaining agreements, and state and federal law. 2. PLAN DESIGN CITY employees are currently offered a HDHP plan. There is a small population of retirees who are offered a PPO plan. Included in this submission is a summary of the current plan designs, in addition to a copy of the current benefit summary’s and summary plan description for a more detailed description of the current benefit programs. Please acknowledge if you are able to match the CITY’s current benefits as closely as possible, and highlight any differences in benefits that cannot be administered.
