Specifications include, but are not limited to: Case management services will include a combination of direct service provision and coordination of, and referral to, services that are targeted to each resident. Case management tasks include but are not limited to the following: A. Submit a detailed People Plan within nine months of grant award. HUD reserves the right to negotiate and must approve the final overall plan as well as each specific project, including defining the target population for services funded by the grant. The plan must address the following priority outcomes in the areas of health, education, income, and employment. B. Develop and implement a resident outreach plan to enroll eligible residents into the case management program. C. Assist all adult residents and their dependents to identify and overcome barriers to economic and housing self-sufficiency; Refer residents to programs that help them obtain and maintain employment, further education or training, and connect to vocational services, community activities, or community service placements. D. Link residents to homeownership preparation education and counseling programs. E. Link children 0-5 years old to high-quality early education services. F. Link school-age children to high-quality education and youth development programs. G. Communicate with residents about supportive services and education programs. H. Leverage resources and develop and maintain partnerships with other service providers in the community. I. Work collaboratively with other key stakeholders in the Transformation effort, including the SNRHA and a range of other partners, to ensure seamless integration of services and supports for targeted families.
