The selected provider will deliver comprehensive post-crisis follow-up case management services, ensuring seamless transitions from crisis intervention to community-based services. The provider will: Engage residents evaluated but not admitted by Crisis Services. Initiate follow-up within 48 hours of referral or pre-discharge when possible. Coordinate services such as mental health, substance use treatment, emergency housing, and other social supports. Track case management activities and maintain detailed documentation to support billing and reporting requirements. Document service outcomes and report case management activities to identify patterns and gaps in service delivery.
