Specifications include, but are not limited to: provide 5,000 physical $10 Visa gift cards to the Department of Health to support the Office of the Commissioner/Office of Public Health in efforts to encourage community members and vulnerable populations. The gift cards will be given out as an incentive for participants who engage in the outreach initiative. NJDOH has been distributing naloxone, fentanyl test strip kits, hygiene kits and other resources for individuals at risk of overdose: A. Gift cards shall be shipped in bulk directly to Michele Calvo in the Office of Opioid Response and Policy at NJDOH 55 North Willow Street, 3rd Floor 3-109, Trenton, NJ 08608. B. Only physical Visa gift cards will be accepted. C. Visa gift cards that do not require phone or internet service to activate are preferred. (Please specify on quote) D. Gift cards must have expiration date of 24 months or longer from disbursement. E. The Contractor shall be prepared to ship up to 2,500 gift cards within fifteen (15) business days following Contract award, the remaining 2,500 visa cards shall be shipped within thirty (30) business days following the Contract award. F. No inserts/letters shall accompany the physical gift cards, gift cards shall not include a specific name or department.
