Purpose: The City of Santa Fe’s Youth and Family Services division collaborates with the Children and Youth Commission (CYC) to provide alternative to detention programs that serve youth at risk or youth offenders. The CYC is tasked yearly with grant management, program planning, service integration, data analysis, and program evaluation on alternative to detention programs that service youth at risk or youth offenders. For FY25-27 the board was awarded a grant to provide alternative to detention services aimed at reducing recidivism through several programs, all of which will be overseen by the continuum coordinator. The continuum coordinator serves as a liaison between the CYFD Grant Manager and the City of Santa Fe and coordinates program activities. The City of Santa Fe will allocate Children, Youth and Families Department (CYFD) funds to the continuum coordinator to oversee the programmatic delivery of alternative to detention services to reduce recidivism in juveniles. Scope of Work: A. The continuum coordinator shall perform the following work: 1) Organize, coordinate and provide staff support for the CYC; this will include board development activities in conjunction with the CYC chair; 2) Inform CYFD’s Program Manager of the date of each meeting and submit a copy of the written minutes of each meeting, within thirty (30) days of the meeting; 3) Coordinate/oversee the programmatic delivery of subcontract requirements of local Continuum service providers; 4) Submit to the City of Santa Fe monthly requests for reimbursement. Such requests will be submitted on CYFD Program Invoice and Expenditure Report forms, signed and dated by an authorized agent of the City of Santa Fe, to ensure that requests for reimbursement are submitted by the due date of the fifteenth (15) day of the following month, unless otherwise approved by the CYFD Program Manager, in advance; 5) Provide data reports as required by the federal government, corresponding to the activities described in this Scope of Work. The CYFD Program Manager will provide the data report format. Programmatic data reports will be submitted monthly to CYFD and must accompany the monthly invoice. Failure to submit such programmatic data and financial reports may result in notice to the City of Santa Fe of non-availability of funds and/or the denial of payment by CYFD. 6) Provide CYFD standardized progress reports monthly;
