Statement of Work:
The contractor will provide related services in Speech/Language Therapy for the period beginning 10/01/2021 - 6/30/2022. Services will include direct therapy to students, consultations with staff/parents, evaluations/assessments, providing documentation/service logs of therapy sessions, guiding speech assistant and participation in IEP meetings. The contractor will provide a direct service log for each student and each visit; and will be asked to meet with the Special
Education Department at the beginning of the contract. The contractor will meet with Special Education Staff and create a plan of action to offer compensatory time to students owed service minutes.
Background:
The John F. Kennedy Day School serves students on the White Mountain Apache Indian Reservation. The scope of work calls for professional related services to assist the school in implementing special education and related services as authorized under P.L. 108-446 Individuals with Disabilities Education Improvement Act (IDEIA) The purpose of IDEIA program is to provide a free appropriate public education to students who have an identified disability and by reason thereof, require specialized instruction including related services to meet their unique educational needs.
The John F. Kennedy Day School serves students on the White Mountain Apache Indian Reservation in a little community in Cedar Creek, Arizona.
Position: Speech/Language Therapy
The contractor will provide related services in Speech/Language Therapy for the period beginning October 01, 2021 - June 30, 2022. Services will include direct therapy to students, consultations with staff/parents, evaluations/assessments, providing documentation/service logs of therapy sessions, guiding speech assistant and participation in IEP meetings. The contractor will provide a direct service log for each student and each visit; and will be asked to meet with the Special Education Department at the beginning of the contract. The contractor will meet with Special Education Staff and create a plan of action to offer compensatory time to students owed service minutes.
Identified Students:
There are approximately 18 students that qualify for Speech/Language Therapy on a weekly basis and approximately 5 more students currently waiting for speech assessments based on referrals. The contractor will provide speech services and evaluations (both initial and re-evaluations) to the students of John F. Kennedy via tele-therapy on the computer. The contractor will service students between the hours of 8 am to 3 pm Monday through Friday, excluding holidays. The contractor will work 5.4 hours each day and 27 hours each week (unless pre-approved to assessments for new referrals or re-evaluations for existing students). The contractor will be available to answer any questions whenever necessary.
Objective:
To ensure the provision of Speech/Language Therapy services to those special education students identified or those referred for possible special education services attending John F. Kennedy Day School. Services will be provided in accordance with P.L. 108-446, the Individuals with disabilities Education Improvement Act of 2004, as specified in individual student IEPs.
1.Observes, screens, evaluates students and provide documentation of student referrals;
2.Participates in IEP meetings.
3.Develops and/or implements therapy plans including a narrative description of students exact needs and specific, measurable individual goal statements;
4.Maintains service to students as specified in IEPs.
5.Identifies equipment needs and makes recommendations of needs to the Special Education Department;
6.Will guide/collaborate with par-educator (aide) for a smoother transition via tele-therapy speech services.
7.Completes student reevaluations and initial evaluations in accordance to BIE timelines;
8.Provides written evaluation reports to the IEP team prior to the team meetings
9.Completes and submits to the Special Education Department written reports of student progress at the end of each school quarter; and
10.Provides training and support as needed to staff and parents.
Professional Staff Required:
The position of Speech/Language Pathologists (service provider) is required to complete this project. The Speech/Language Pathologists must be licensed in the State of Arizona, with a Masters Degree in Speech/Language Pathology.
The Speech Pathologist will maintain National Certification through the American Speech and Hearing Association as well as a medical license through the Arizona Department of Health, and a Valid Arizona Drivers License.
The service provider is to;
1.Submit all documentation of services and reports to the Special Education Department.
2.Collaborate with the Special Education Coordinator to establish monthly schedule.
3.Complete evaluations/assessments for Three-year evaluations and new referrals.
4.Meet with Special Education Department on the first day of work to organize structure for service provision and procedures delineated for providing specified services.
5.Compete all required DOI Trainings.
6.Comply with confidentiality and FERPA laws. The Family Educational Rights and Privacy Act (FERPA) (20 U.S.C. § 1232g; 34 CFR Part 99) is a Federal law that protects the privacy of student education records.
Period of Performance: Services are only provided during the school year and Extended School Year (approximately September through June).
The Contractor or any sub-contractor on-site employees are required to obtain a positive result on all background investigation prior to the implementation of services.
The Contracting Officers Representative (COR) shall coordinate all activities for the background investigation.
