Contractor and clinicians will provide client-specific services to adults incarcerated in the Bernalillo County Metropolitan Detention Center (MDC) booked on the following charges: DWI, substance use, and other charged deemed to be specific to substance dependent related charges deemed appropriate and court ordered by the judicial system. Adults in the target population are those incarcerated and experiencing substance use disorders. 1. Services offered should include but not limited to clinical care including individual assessment, group facilitation, care coordination, case management, patient data management and discharge planning. 2. Individual assessment for each inmate referred to the program shall be determined through a comprehensive assessment and individualized service plan to meet each individual’s needs and treatment goals. 3. A multidisciplinary treatment team approach which will monitor client progress toward goals and modify the plan accordingly will be implemented. 4. Referring individuals to Intensive Outpatient Services to continue services once released from MDC. 5. A monthly report on the numbers of clients in the service matrix will be provided to the County. 6. The Contractor will collaborate with the County in determining appropriate outcome goals for clients and will assist in gathering data for reporting purposes. 7. Participate in a yearly evaluation conducted by a vendor of the County’s choice to determine fidelity of the program, program outcomes and cost analysis.
